REOrthopaedics, Inc., is located in Southern California, however, our orthopedic surgeons treat patients from anywhere in the United States, Mexico, Asia, and across the globe. We ask that you read the information below, as it provides a detailed description of what our orthopedic surgeons expect from the patient before the initial consultation at our San Diego practice. Internet Screening Program

Our San Diego orthopedic surgeons, George Cierny III, M.D. and Doreen DiPasquale, M.D., have dedicated their careers to the evaluation and treatment of musculoskeletal disorders. They are committed to helping every man or woman contacting their office and have assisted patients from all over the globe. However, to answer questions over the Internet, they will need specific information pertaining to your condition.

Please complete our web-based information form via the link below and click submit. Once we have had an opportunity to review the forms, one of our surgeons will contact you at the e-mail address provided. Please understand that a short series of questions may be necessary to gather enough information to adequately address your concerns.

If your case is complicated and/or more specific information is needed to respond judiciously, a more traditional consultation may be indicated (see Formal Consultation Form ).

Formal Consultation ($200.00)

There is a nominal fee for this level of evaluation due to the time and effort involved. Following an informal screening (described above), our doctors will list whatever information they might need to facilitate the process. You may be asked to provide:

• specific medical records and documents
• hard copies and/or digital photographs of diagnostic studies (i.e. x-rays, MRI and/or CT scans, nuclear scans, etc)
• clinical photographs of a wound or injured part
• a more detailed Patient Information Form (below)

Should the surgeons at REOrthopaedics Inc., feel you are a candidate for treatment at their Southern California center, they will send you a detailed list of treatment options (methods and outcomes), an estimated cost of treatment and several affordable ways to finance care. If it is determined you are not a candidate for treatment, an explanation will be provided. The $200.00 consultation fee is non-refundable.
